The Surgident Osseodensification Sinus Expansion Drill (SED) Kit is a precision-engineered osseodensification system designed for safe and predictable crestal approach sinus floor elevation in dental implant surgery. The kit combines a sequence of five sinus expansion drills (2.5 to 4.2 mm) with seven depth-control stoppers (4 to 10 mm), allowing the surgeon to incrementally expand the osteotomy while gently lifting the Schneiderian membrane through controlled hydraulic pressure rather than cutting. The bone-preserving design conserves and densifies the residual ridge instead of excavating it, maintaining the critical bone volume needed for implant primary stability. Directions for Use

Pre-Surgical Planning

• Measure the depth of the residual bone of the maxillary sinus through pre-operative X-ray or CBCT imaging
• Plan the implant position, diameter, and required sinus floor elevation height
• Verify the case is suitable for crestal sinus elevation (typically 4-8 mm of residual ridge height)
• Confirm the patient has no contraindications such as active sinus pathology, immune deficiency, or relevant systemic conditions

Kit Preparation

• Sterilize the complete kit via autoclave following the recommended protocol (121°C / 30 min or 132°C / 15-25 min)
• Open the sterilized cassette and verify all drills, stoppers, and accessories are present
• Arrange the drilling sequence — guide drill first, then expansion drills in ascending diameter order

Guide Drilling

• Attach the Ø2.0 mm guide drill (SD-GD20) to a compatible surgical motor handpiece
• Set the rotation speed to 800-1200 RPM with copious saline irrigation
• Drill the pilot osteotomy to 2 mm shorter than the measured residual bone depth
• Example: if measured residual bone depth is 6 mm, use the number 4 stopper (SDST-04) to limit guide drilling depth

Sinus Expansion Drilling

• For safety, mount a stopper one number lower than the planned final drilling depth on the sinus expansion drill
• Example: For 5 mm depth drilling, mount the number 4 stopper (SDST-04)
• Begin with the smallest sinus expansion drill (SED-25, Ø2.5 mm) at 800-1200 RPM with copious irrigation
• Drill until the stopper reaches the alveolar crest — this safely limits drilling depth
• Progress through the expansion drills in ascending diameter (SED-28 → SED-33 → SED-37 → SED-42) per the planned implant diameter
• The drills incrementally expand the osteotomy while preserving bone through the osseodensification effect

Progressive Depth Extension

• If drilling is not complete when the stopper reaches the alveolar crest, replace the stopper with one of higher number and continue drilling
• Example: If the number 5 stopper (SDST-05) is not enough, switch to number 6 (SDST-06) to drill deeper
• Progress the depth gradually to gently lift the Schneiderian membrane through controlled hydraulic pressure
• Avoid sudden deep drilling — the controlled stopper-based approach is essential for sinus floor safety

Sinus Floor Elevation and Graft Placement

• Verify the sinus floor has been lifted to the planned height without membrane perforation
• Place the chosen bone graft material into the prepared site to fill the elevated compartment
• Proceed with implant placement using the implant manufacturer’s protocol

Post Use Care

• Disassemble all components and rinse with flowing water to remove body fluids and debris
• Clean each drill and stopper thoroughly using suitable brushes (do not use metal brushes or steel wool)
• Use a surgical instrument cleaner per the cleaner manufacturer’s instructions
• Rinse three times in flowing or distilled water for 20 seconds each
• Air-dry or wipe with clean cloth and alcohol to prevent water spots
• Sterilize via autoclave or EO gas per the recommended protocols
• Return all components to their designated positions in the cassette for storage
