Directions for Use

Preparation

• Evaluate the residual bone height below the sinus floor using CBCT before planning the procedure
• Confirm the Aqua TOCA technique is suitable based on bone height and membrane condition
• Inspect all kit components for integrity and autoclave the complete kit before surgical use
• Prepare sterile saline in a syringe connected to the silicone tube for hydraulic delivery
• Reflect a full-thickness mucoperiosteal flap to expose the implant site in the posterior maxilla

Site Marking and Drilling

• Mark the surgical site at the planned implant position using the guide drill
• Select the appropriate stopper based on the planned drilling depth and engage it onto the SD-Reamer
• Drill with the stopper engaged all the way until the reamer reaches more than 1 mm beyond the membrane
• Operate the SD-Reamer at 800 to 1200 RPM with copious saline irrigation throughout drilling

ALS Fixation and Hydraulic Lift

• Select the appropriate ALS diameter (3.0 or 3.5 mm) matching the prepared osteotomy
• Fix the ALS into the perforated hole manually using the wrench or with the handpiece condenser
• Operate the handpiece at 30 to 40 RPM with maximum 20 Ncm torque if using engine-driven insertion
• Connect the silicone tube and saline syringe to the ALS and inject saline slowly
• Allow hydraulic pressure through the five aqua holes to gently lift the membrane into a dome shape

Graft Placement and Implant Insertion

• Remove the ALS once adequate membrane elevation is confirmed clinically and radiographically
• Fill the elevated sinus space with bone graft material using the bone syringe
• Condense the graft material gently using the bone packer for uniform distribution
• Insert the dental implant fixture of planned diameter and length following standard placement protocol
• Verify primary stability and seat the cover screw or healing abutment as per the case plan

Post Use Care

• Suture the flap for tension-free primary closure and verify hemostasis
• Prescribe antibiotics, decongestants, analgesics, and sinus precautions for the patient
• Clean all kit instruments thoroughly in an ultrasonic cleaner with enzymatic solution
• Inspect cutting edges of the reamers and replace worn components as needed
• Autoclave and store the complete kit in the supplied tray for the next surgical use
