Directions for Use

Select the Crown Form:

• Choose the appropriate size premolar crown form from the tray

Cut if Necessary:

• If the crown is attached to a plate, carefully cut it off using scissors

Puncture the Crown:

• Make a small hole in the crown to allow excess composite material to escape during placement

Fill with Composite:

• Load the crown form with the desired light- or chemically-cured composite material

Place on Tooth:

• Gently press the filled crown form over the prepared tooth

Remove Excess Material:

• Wipe away any excess composite that extrudes from the puncture hole or margins

Cure the Composite:

• Light cure through the transparent form, or wait for the chemical cure to set

Remove the Crown Form:

• Once the composite is fully cured, peel off the crown form carefully

Finishing and Polishing:

• Perform minimal contouring if needed; polish the restoration for a smooth finish
